Position Title:Community Health Advocate I (Bridging the Gap)

Department:Healthy Communities

Reports to:Program Manager, Community Health

FLSA Classification: Non-Exempt

Compensation:$25.00 - $27.00/hourly

Background:

Fifty years ago, four Black American mothers, aided by 25 volunteer Black physicians, launched Baywell Health, formally known as West Oakland Health amid the Civil Rights and Black Power movements. It immediately became an anchor for the Black communities in West Oakland, East Oakland, and Berkeley for healthcare, health education, food and housing programs, and voter registration. The clinic became a spark that transformed healthcare advocacy in California and the nation.

Baywell Healths purpose is to be the trusted hub advancing the Bay Area Black communitys health and dignity. We work to do this by providing high-quality,comprehensive adult and family medicine, pediatric health care, dental care, optometry, podiatry, behavioral health services, and pharmacies at two locations while also being a space where medical care is our central but not the sole aim. We aim to be a place where community members also receive assistance on things that will make our community socially, economically, emotionally, and physically healthy.

Job Summary:

Under the general supervision of the Community Health Program Manager, the Community Health Advocate I works to conduct outreach that identifies with unmet social support needs, provides community resource and referral assistance to patients and community members, and advocates for the coordination of healthcare and social support services for those individuals.

Duties/Responsibilities:

  • Identify patients with unmet social determinants of health needs using screening assessments to identify homelessness, self-care disability, food insecurity, low literacy, language preferred not English, needs, access to educational, economic and job opportunities, public safety, social support, exposures to crime, and violence. This will be documented in the medical chart and will be reviewed to identify patients who require higher levels of care management.

  • Conduct community assessment/screenings to identify disparities in unmet social needs and common health issues affecting individuals.

  • Assist community members with applying to Medi-Cal specifically in settings such as health fairs and other community-based events. CHA will serve as part of an integrated health care team to coordinate health services, assist with referrals, and ongoing care coordination.

  • Conduct outreach to promote healthy lifestyles and behaviors to targeted populations. This includes, but is not limited to, conducting MyChart outreach, providing chronic condition education, initiating new member engagement, contributing to member retention efforts, and participating in other WOHC health promotion events and activities.

  • Actively engage with community members while implementing health programs by encouraging active participation from individuals and requesting feedback for continuous improvement.CHA will shadow CHA II staff attend meetings with county-wide agencies to coordinate health services for low-income families.

  • Will attend and actively participate in training and care coordination meetings. This includes, but is not limited to, assisting with other Community Health programs and providing back up for other CHAs or West Oakland Health Council (Council) team members.

  • Performs other related duties as assigned.

  • May be assigned cross departmental responsibilities.

  • Basic computer skills (Excel, Microsoft Word).

  • Ability to project a positive image to the public.

  • Bilingual proficiency is highly desirable.

  • Excellent verbal and written communication skills.

  • Excellent interpersonal and customer service skills.

  • Excellent organizational skills and attention to detail.

  • Excellent time management skills with a proven ability to meet deadlines.

  • Ability to function well in a high-paced and at times stressful environment.

  • Must possess a valid drivers license.

Education and Experience:

  • High school diploma or equivalent (required).

  • Bachelors degree in public health, health education, or related field (preferred).

  • All CHAs will participate in training to become certified Community Health Workers (CHWs). All CHAs will participate in training to become Certified Enrollment Counselors. CHAs will be expected to seek and complete other health professional development training courses as appropriate.

  • 1-2 years in a community health setting or equivalent education/experience combination (required).

  • Experience working with underserved, minority populations (preferred).

  • Environmental and Health Hazards: Frequent exposure to communicable diseases and other conditions common to a clinic environment.

  • The employee must occasionally bend, lift and move up to 25 pounds.

  • Specific vision abilities required by this job include close vision, peripheral vision, and the ability to adjust focus.

  • May be required to work in an environment with high noise levels and unpredictable temperature and ventilation.
